I'm not saying that they shouldn't question the results if they feel that there's cause for concern, but a the same time all the clean eating and exercise in the world isn't going to stop a genetic predisposition to something like high blood calcium, which has been linked to coronary artery disease. You can reduce the risks, but you'll be hard pressed to completely outrun it. Exactly

Sudden deaths should have an autopsy (unless for religious reasons & even then there should be one!)

The cause of death is the most commonly sited cause of death when there is no autopsy. It would be based on external examination and maybe blood tests. It may or may not be right, but if the family didn't want an autopsy, and there was no reason to suspect anything but natural causes, then they're pretty much stuck with what the death certificate says. If they wanted an accurate answer, then they needed to have an autopsy done.

When my mom died, we didn't know the cause. I was guessing either stroke or heart attack. When I talked to the County Medical examiner, I told her I wanted an autopsy. She said she wouldn't order one until she went through my mom's medical records to see if she could find anything that would provide an explanation. I told her mom's medical history was clean, and she wouldn't find it. She didn't and I got my autopsy. But if she said no, I could have paid for it myself, which I probably would have.
My mom had a widowmaker heart attack. A slight split in the artery at the base of the heart caused the blood to be pumped into the sac lining the heart. She probably never felt the split. She could have been walking around with it for 2-3 days. The ME said it was unavoidable, untreatable and undetectable. My mom had low BP and did light exercise. I'm guessing Lance had a similar fate.

We had to wait for the results of an autopsy before we could arrange my stepdad's funeral. It was standard practice because it had been a certain amount of time since he'd last been seen by a doctor and passed away at home.

No one in my family has ever been autopsied, besides my great aunt murdered by her ex husband (my maternal great uncle) back in the mid-60s and that was for the judicial process. 3 shotgun blasts the abdomen is pretty straightforward for cause of death.
Some died at home and some in the hospital post traumatic medical event. It’s not often standard procedure in the States unless the family asks or the death seems odd or suspicious.
